This GM is actually building this entire world. It's enormous, and meticulously cultivated. He has a giant world anvil site that he has been building for years. The actions of his players have permanent effects on the world for future games, so the lore is constantly getting polished and refined. The maps are all custom creations that he has built, and he has a ton of artwork that he uses whenever we encounter new characters or situations.
