This is actually a super hard question to answer! The Vorkosigan Books by Lois McMaster Bujold are certainly up there, though.
How Beroli runs games
As a GM, I believe in rule of cool. I'm also a firm believer in logic over the letter of the rules in cases where there is a conflict. (Drowning does not heal, for one infamous example from D&D.) I prefer character-centric games.
